• Letters of recommendation:
  • Two (2) science faculty where you have taken a class and earned a grade
  • One (1) general dentist (non-relative, practicing in the US or Canada)
• Minimum 60 at the time of SEP application: 
  • Students who are admitted to UDM School of Dentistry will be required to complete the additional 40 hours prior to matriculation 
• Personal statement (AADSAS prompt accepted):
  • Why have you decided to pursue a career in the dental profession?
  • What are your goals as a dental professional?
  • Explain your desire to participate in the Summer Enrichment Program and why you are a great candidate for this program.
  • Describe your experience while shadowing staff at a dental office and/or clinic.
  • Discuss your commitment to providing care in underserved communities. This may include any personal life experiences you have had involving health issues in an underserved or disadvantaged area. Explain how these have motivated you to pursue a career in dentistry
• Disadvantaged background statement
  • In an essay of 1,000 words or fewer, please provide details explaining your economically and/or educationally disadvantaged status if you qualify in one of these categories. This should be typed into the online application. If you do not qualify in one of these categories, this statement is optional.
• DAT scores (if available; self-reported scores accepted)
